Prise en charge des patients sous chimiothérapie en chirurgie buccale

Abstract

Le nombre de nouveaux cas de cancers étant en augmentation, les médecins-dentistes son donc amenés à prendre en charge de plus en plus de patients sous traitements médicamenteuanticancéreux (chimiothérapie, bisphosphonates, hormonothérapie, immunothérapie et/ou thérapies ciblées). Ces thérapies induisent divers effets secondaires au niveau de la cavité buccale notamment les mucites, la xérostomie, la dysgueusie et l’ostéochimionécrose ; dont la gravité est variable. L’odontologiste joue un rôle primordial dans la prise en charge de ces patients et doit assurer en aval une démarche préventive afin de minimiser le risque de complications. Un suivi régulier doit également être instauré tant durant les traitements qu’une fois ces derniers terminés. Le médecin-dentiste fait donc partie intégrante de l’équipe soignante et se doit de connaître parfaitement la conduite à tenir, ainsi que les précautions particulières à prendre lors de la réalisation de soins bucco-dentaires chez ces patients. Abstract: The number of new cases is increasing; the dentists are leaded to manage patients with anticancer drugs (chemotherapy, bisphophonates, hormonotherapy, target therapy and immunotherapy). This therapies induce various oral side effects especially mucositis, xerostomia, dysgueusia and osteonecrosis of the jaws (ONJ) whose the severity is variable. The odontologist plays a primordial role in the managing of these patients and has to insure preventive approach in order to reduce the risk of complications. A regular follow-up had to be established during the treatment and after finishing. The dentist is an integral part of the healthcare team and he has to know perfectly the management, and the special precautions to take with these patients during the dental care.
