Approche Orthodontico-chirurgicale des dents incluses

Abstract

L'inclusion dentaire, est la rétention de la dent dans l'os au-delà de sa date d'éruption entourée par son sac folliculaire. Le praticien doit être en mesure d'expliquer à son patient les différents problèmes engendrés par l'inclusiop et les différentes propositions thérapeutiques qui s'offre à lui. Un bon examen clin que complété par les différentes explorations radiologiques permet de poser un diagnostic précis et choisir l'attitude thérapeutique appropriée. Le choix du traitement se fait de l'abstention et la surveillance de la dent incluse à l'avulsion de celle-ci, on peut également avoir recours à des procédés chirurgicaux purs qui favorisent l'éruiption spontanée de la dent ;mais la technique de choix est le traitement orthodontico-chirurgical qui est un acte pluridisciplinaire , nécessite la collaboration entre l'orthpdontiste et le spécialiste en chirurgie buccale pour accéder à la dent incluse et la tracter, en assurant son égression dans un parodonte sain. le traitement orthodpntico-chirurgical comprend un temps orthodontique préchirurgical qui vise aménager l'espace nécessaire pour la mise en place de la dent incluse et préparer l'unité d'ancrage ,un temps chirurgical dont le but est de supprimer les obstacles de l'éruption, le dégagement de la dent incluse pour permettre le collage de l'attache et un temps orthodontique post-chirurgical qui permet de tracter la derit incluse ; cependant ; des complications peuvent survenir à plus ou moins long ternie , des contrôles seront nécessaires. Abstract : The dental impaction isl the retention of the tooth in the bone beyond its eruption date surrounded by its follicl. The practitioner must be able to explain to his patient the different problems produced by the the clental impaction as well as the different therapeutic offers. A good clinical assessment done using different X-ray images allow to make a precise diagnosis and to chooe the most suitable therapeutic attitude. The chosen treatment is made from the abstention and the observation of the impacted tooth to its ivulsion, we can also have recourse to pure surgical methods that foster the spontaleous eruption of the tooth, but the chosen technique and the surgical orthodontic treatment is a multi-disciplinary act that requires a collaboration between the orthodo tist and the periodontist surgeon to have access to the impacted tooth and to ow it, by assuring its extrusion in a safe periodontium. The orthodontic treatrient includes an orthodontic preoperative period that aims at setting up the required space to place the impacted tooth and prepare the dental implant unit, the aim a:if a surgical period is to eliminate any outbreak impediments, digging out the impadted tooth to allow the brace collage and an orthodontic post- operative period wiiich allows towing the impacted tooth, however, some complications may happen in the long or short term , medical checks will be necessary
