Résumé:
Les patients ayant un trouble de l’hémostase, consultant en odontostomatologie, peuvent être connus et
suivis pour leur pathologie, ou méconnus, en raison de l’absence de toute manifestation clinique de ce trouble.
Le geste chirurgical (extraction ou autre), fait sans aucune préparation et l’hémorragie qui s’en suit, est alors le
signe révélateur. L’odontostomatologiste doit de ce fait, avoir le réflexe d’examiner toute la cavité buccale, à la
recherche de tout signe pouvant orienter vers la suspicion puis la confirmation d’un trouble de l’hémostase.
La prise en charge de ces patients ne peut se faire qu’avec l’étroite collaboration du médecin traitant, qui
doit nous renseigner sur une fiche navette sur la pathologie de l’hémostase.
Le risque étant surtout un risque hémorragique, la prise en charge doit se faire en tenant compte de ce
risque qu’il faudra prévenir par l’application systématique des techniques locales d’hémostase, faciles a mettre
en œuvre, non onéreuses et surtout de réalisation possible même par l’omnipraticien. Dans certains cas, dans
les déficits sévères, le recours a un traitement complémentaire par voie générale (substitution de facteurs et
des fractions manquantes), devient indispensable. Mais le meilleur traitement de l’hémorragie reste sa
prévention.
Summary:
In dental practice, patients with haemostasis disorder can be known and monitored for their pathology, or
maybe unknown, due to the absence of any clinical manifestations of this disorder. A surgical act (dental
extraction) done without preparation may cause unexplained bleeding, which can be the revealing sing. The
Odontostomatologist must therefore in this case have the reflex to inspect the hull oral cavity, in order to search
for any sign that can let us suspect then confirm the presence of hemostasis disorder.
Taking these patients in charge is possible only with the close collaboration with the treated doctor who
controls the patient’s medical condition, to gives us informations about the pathology.
The major risk is the hemorrhagic risk, the management of the dental health cares must be done with
taking in consideration this risk, which we have to prevent by the systematic application of local hemostasis
technics, easy to use and to manipulate, not expensive and the most important the possibility of using them by
the dentist. We have to remember that in some cases, in the severe deficiency the recourse to a complementary
treatment by the systemic way (substitution of factor and missing fractions) becomes indispensable. However
the best treatment of hemorrhage is the prevention.
