Résumé:
Dans les situations physiologiques normales, un équilibre entre l'activation et la limitation de
l'hémostase prévient à la fois la thrombose et l'hémorragie. Les AVK, dont l'anticoagulant de choix en
Algérie est le SINTROM®, sont utilisées dans le traitement préventif ou curatif des accidents
thromboemboliques. Cette utilisation pose le problème majeur des hémorragies qui peuvent être
prévenues par un suivi étroit de l'INR (compris entre 2 et 4,5 selon la cause du traitement).
Nous avons effectué un travail à la fois clinique et biologique, qui consiste en une étude
descriptive pendant trois mois, chez 32 patients de l'UMC Frantz Fanon Blida, sur les cas de surdosage
aux AVK, dont 33% d'INR d'admission était supérieur à la cible, 60% en revanche, était dans ou
inférieure à la cible. Parmi ces patients, 41% ont un âge supérieur à 70 ans, 41% prennent leur traitement
depuis plus de 5 ans, avec 48% d'interaction, représentée par les antiagrégants plaquettaires, l'hématome
étant la complication la plus répandue. La prise en charge varie selon l'INR d'admission et la gravité du
saignement.
En conclusion, ces complications sont dues à la mauvaise adaptation du traitement et aux
interactions médicamenteuses. La prescription des AVK nécessite une optimisation thérapeutique
incluant l'éducation du patient et la bonne prise en charge par le clinicien.
Abstract:
In a normal physiological context, an equilibrium between the activation and limitation of
homeostasis is crucial in preventing both thrombosis and bleeding. AVKs, of which SINTROM® is the
most prescribed in Algeria, are used as preventive or curative treatment of thrombo-embolic accidents.
The problem of bleedings arises with this wide usage, which can be prevented through a strict follow up
of INR (included between 2 and 4.5 according to the cause of the treatment).
We realized a three-month descriptive study on 32 patients from Frantz Fanon Emergency
department, about AVK overdose cases, of which 33% of admission INR was superior to the target,
60% being in or inferior to it. Amongst these patients, 41% are above 70, 41% take their treatment for
more than 5 years, with 48% of interaction, represented by antiplatelet drugs, hematoma being the most
spread complication. The canes provided differ according to the admission INR and the seriousness of
the bleeding.
In conclusion, these complications are due to the bad treatment adaptation and drug interactions.
AVK prescription requires good therapeutical optimisation that includes patient education and high
quality care delivered by the practitioner.
