La Chirurgie endodontique : Ultime recours de l’endodontie

Abstract

La microchirurgie endodontique occupe aujourd’hui une place primordiale dans l’arsenal thérapeutique du chirurgien dentiste permettant de prolonger la durée de vie d’une dent autrement condamnée sur l’arcade. Les objectifs de la chirurgie apicale sont alors d’éliminer le tissu mou pathologique, réséquer l’apex, et réaliser une obturation à rétro étanche et biocompatible. L’introduction du microscope chirurgical et des instruments ultrasoniques et de microchirurgie ont apportés la possibilité de réaliser une préparation non délabrante sur le plan dentaire et osseux, de plus de nouveaux matériaux d’obturation sont introduits en place sur le marché, tel que le MTA ® qui permet de garantir l’étanchéité de l’obturation à rétro et même une cicatrisation apicale. La chirurgie endodontique est maintenant bien codifiée, son taux de succès se rapprochant de celui de l'endodontie conventionnelle. Summary : Endodontic microsurgery take an important place in the endodontic treatment. The main aim is to maintain tooth on the dental arch. The objectives of apical surgery consists of suppression of the pathological soft tissue, resect the apex, and provide an adequate apical seal. The introduction of the dental operating microscopes, the ultrasonic and microsurgical instruments have made it possible by reducing the removal of healthy bone and dental structure. Moreover, new filling materials are introduced into the market, such as the MTA ®, with excellent sealing ability, biocompatibility and even apical healing which improves the prognosis. Endodontic surgery is now well codified, with a success rate close to that of conventional endodontic.
